Apparently what some hackers see:

Punkbuster scans your files for anything that modified,if it finds something supicious then it kicks you from the current server and if it finds something that looks like a hack the it GUI bans you.
Currently most hacks run separate to BF3 (like an overlay) and instead of modifying the files of the game it reads them,makes a copy then modifies them and displays them in the overlay in a way to give the hacker the information he needs (the original files still stay the same)
If Punk buster was allowed to have access to more files on peoples PC's then it would have been able to pick up 100% of hacks but unfortunately (and fortunately for your porn collection) it goes against the privacy law or something like that.
You can do anything if you put your mind and will behind it,provided it's within the bounds of logic and you have allot of time.
