    Arma 2 Combined OPS, with ACE and ACRE mods (there are more, but that is as real as it gets without actually bleeding)

    yup ArmA 1 or 2, love these games. Flippin hard though but worth it!

    One shot will usually kill you or cripple you at least and it is often difficult to locate where you're being shot from. No radar here.

    I also thought Flashpoint Dragon Rising made a fairly good attempt at being realistic. except for the bandaging bullet wounds in your head part.

    As stated before, ArmA 2 - The single player is horrible but if you play with a community nothing beats it. Currently arma.co.za is the only active local community.

    Arma II is very realistic. And Unforgiving. Frustratingly unforgiving. A bit too much for me, but hey, if that's what you're looking for...

    For realistic shooters I used to enjoy the old Ghost Recon (not that new nonsense) and some of the older Rainbow Six games can be good. I'm not sure how many are still playing them online though.

    Though if you haven't played a realistic shooter like Ghost Recon prepare to be turned into cannon fodder, people used to the modern shooters will be lucky to get 1 or 2 kills in those (including suicides lol).

    I bought Rainbow Six: Covert Ops a long time ago, this is still one of the most fun shooters I have played. Really dated, but AWESOME.

    Ghost Recon Future Soldier does a good job of balancing realism with fun factor.

    I'm going to agree with all that the best bet is Arma II, great game if you are looking for a realistic FPS indeed.
