its when you first log in ?
it happens when you either ALT+Tab out of the game during login, or when you playing in windowed mode and minimise the window during login.
it is best to not interupt the game at all while loging in, keep the window active, and once you are logged in, then go about alt tabbing or whatever it is you wish to do.
