for submissions you have two ways of getting out: the technical way which puts you in an advantage position after wards and the brute strength way which usually end up you going back to a standing position. rotating the right thumb stick clock wards is the technical way(use it with fighters that have a high sub defense rating) mashing the face buttons is the brute strength way(use it when your fighter has a high strength rating) I found that the brute way is easier but the tech way is more advantageous.
As for countering ground moves....watch your opponent, when he starts moving to do the transition, tap the right thumb stick left or right(the timing is tricky but easily learned) If done right you'll counter him and end up in a better position without losing stamina and he'll loose stamina(perfect strategy to get a submission on your opponent) if you struggle with counters then just keep the right thumb stick to the left or right. If he tries to transition while you doing this, his attempt will fail( sometimes if his ground skills are a lot higher then yours he'll succeed) these failed attempts will also deplete his stamina.
Hope this helps you a bit. Just play a lot, that's how I learned...
Enjoy
Or you could just practice in real life...
