Wirelessness. I'm a simple man with simple needs, like being able to move my mouse where I want it.
 
Ergonomics! The mouse definitely have to be comfortable if you're going to sit on a PC for hours on end. Be it for work or play "whistling:
 
whats the point of having all those features if its not any more comfortable than your 5 year old mouse, for me before anything else it has to be comfortable in my hands
 
Granted, an extra button (for eg. knifing/melee) would be cool. But ergonomics is most important to me. One of the main reasons I have yet to invest in a gaming mouse. As I am about to pull the trigger on a R500+ mouse, I come across the odd review where the guy explains how it made him feel like throwing up... lol
 
I have to follow the pack on this one, Ergonomics is incredibly important to me. I use my mouse for hours every day and it needs to be comfortable.
I've always stuck to Logitech, 2 x G5's, 1 x G9, would definetly buy again in a heartbeat.

Love the frictionless wheel in the G9!

I left my wireless mouse at home this morning and had to borrow a wired mouse for the day. It was a real eye opener. I missed the forward and back buttons. I missed the freedom of movement. I missed the left and right scroll on the wheel as well as the smooth scrolling it allows.
 
I vote ergonomics.

Having a fancy mouse is all good and well, but, having comfortable mouse is what I'd assume all gamers would prefer.
Side button are a definite must. I only have 2 clicks and a scroll wheel. Tricky situations with this mouse can sometimes leave me in the dark. Size, weight and shape definitely become a factor for gaming mice.

I said sensor technology... because accuracy and smooth use is key.

I have had mouse pointers that jumped all over the place, even on good surfaces.
 
I prefer comfort overall, I have big hands and finding a comfortable mouse is difficult unless I pay for a quality mouse :) My Logitech G5 broke after a couple years use, and now using a cheepo Gigabyte mouse that came combo'd with a keyboard hehe.

Same here - wireless with the functions above - wired mouses is too restrictive by far
