Yip. Weird Dreams. Played on PC but it was available for Amiga as well. Probably on other platforms also. A mix between action and adventure, you played a patient on an operating table. The game is the "weird dreams" the patient have while being operated on. Each time you die, the patient's heart stops and he gets resuscitated. Third time, game over.

Regarding the fish flying in the desert: they are sword fish. You jump up, grab a fish and then use it as a sword later in the game.
