Had success inside the refund window, I think twice. Had once tried a refund outside, I was like a few minutes over the allowed time to play, was unsuccessful.
 
Don't think Arkham Knight counts, as that was an exception on the "outside automatic refund window" scenario :p
 
I've never actually tried getting a refund but its good to know that the option is there. Although it does seem to have affected the steam sales slightly.
