This is a tooth extractor. Luckily, this should no longer be in use in the modern detist's office. Commonly dubbed a tooth key, this one is probably a quite early nineteenth-century example, since the shaft is straight. Later, the shafts of tooth keys were curved or angled to avoid breaking adjacent teeth.

The procedure for extracting a tooth using this key was simple: the dental practitioner would place the hooked end of the instrument around the patient’s offending tooth, and with one quick twist of the handle, break the tooth off its roots. Prior to the use of anesthetics, speed was of essence in this process. Happy moments.
